Our Services
Our services here at Revive Home Healthcare consist of a variety of options that are geared towards making each client feel as much comfort as possible. Here are some of the services and treatments we offer here at Revive Home Healthcare.
Professional nursing services are provided in accordance with the plan of care and are case managed by licensed Registered Nurses who may also be the visiting nurse assigned. Additionally, case managers may assign LPN’s who service cases under the supervision of the RN and assist in the implementation of the plan of care.
Physical therapy services are provided in accordance with the plan of care and are case managed by qualified, licensed physical therapists who may act as the primary case manager in the absence of the nursing discipline. Additionally, PT case managers may assign PTAs (Physical Therapy Assistants) who service cases under the supervision of the PT and assist in the implementation of the plan of care. PT may develop rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.
Occupational therapy services are provided in accordance with the plan of care and are case managed by qualified licensed Occupational Therapists (OT). Additionally, OT may assign qualified OTAs (Occupational Therapy Assistants) who service cases under the supervision of the OT and assist the OT in the implementation of the plan of care. OT may develop rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.
Speech Therapy services are provided in accordance with the Plan of care and are provided by a qualified licensed speech therapist. ST may develop rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.
A certified nursing assistant (CNA) provides varying levels of care and support to physically and/or mentally disabled individuals who are unable to care for themselves. CNAs care for patients of all ages in hospitals, inpatient mental health facilities, and clinics.
Infusion therapy involves the administration of medication through a needle or catheter. It is prescribed when a patient’s condition is so severe that it cannot be treated effectively by oral medications. Typically, “infusion therapy” means that a drug is administered intravenously, but the term also may refer to situations where drugs are provided through other non-oral routes, such as intramuscular injections and epidural routes (into the membranes surrounding the spinal cord).
Infusion therapy is also provided to patients for treating a wide assortment of often chronic and sometimes rare diseases for which “specialty” infusion medications are effective.
What Are Skilled Nursing Services?
Skilled professional nursing services include skilled nursing services, physical therapy, speech-language pathology services, occupational therapy, physician services, medical social work services.
Skilled professional services are authorized, delivered and supervised by health care professionals who meet the appropriate qualifications, which are specified under CMS 484.115 (personnel qualifications).

If services include use of the client’s personal funds:
The Agency will obtain an authorization from the client/responsible party, for access to personal funds when grocery or other shopping services are to be provided and when those services include assistance with bill paying or any activities, such as shopping, that involve access to or use of client funds.
